It’s not possible that the process terminated normally with the flush_lock in place. Either there was a hard shutdown/restart of the computer or the process aborted/died or was killed. Enabling write flushing improves fault tolerance (at the significant expense of performance) in the case of uncontrolled shutdown (only) but does not improve reliability.
