So this design is used to enumerate txs with the same hash but different content (how we manage mutability) and also multiple instances of the same tx - such as the two valid bip30 exceptions that may be written in a concurrent block race resulting in two duplicated entries. So we just enumerate these and check each for block association (implying confirmed). Then we just enumerate the outputs of each confirmed instance and ensure all are spent.
